The Mission Cincinnati offers limited benevolence in response to situations of need faced by regular attenders of our church. To request benevolence, this form must be filled out by the head of applying household. The age of applicant and signee must be 18 or older.

*The Mission Cincinnati will not dispense cash gifts under any circumstances.
